Administration of medicines on your behalf
If your child requires prescribed or other medicine to be given during the school day, our trained staff are able to do this on your behalf. You will need to complete this form and hand it to the school office with the medication. Hard copies of this form are also available from reception. Please always provide medicine in its original packaging with the dosage instructions and your child's name clearly visible.
